A: You must submit supporting documents to verify the points claimed during your last CCP cycle. Acceptable documentation includes:
- Certificates or transcripts of healthcare-related courses/training completed during the last certification cycle.
- Each document must include the following information:
- Name of the organization/CE provider
- Title of the course/training
- Date of completion/activity
- Amount of credit awarded (e.g., CEU, CME, contact hours, or credit hours)
